Maama Namu, a life-saving service, provides emergency medical response and transportation to those in need. This concept note outlines the development of a mobile and desktop app to enhance Maama Namu's services, improve user experience, and create a sustainable business model. Service Details • Coverage Area: Phased rollout, starting in greater Kampala and expanding nationwide. • Response Teams: EMTs, paramedics, and hospital staff. • Dispatch Protocol: Emergency calls placed via 9114 or the app trigger a • response. Call center assesses the situation and dispatches EMTs or • ambulances based on urgency. • Criteria for Response: Determined by the nature and severity of the • emergency. Target Users and Functionalities Primary Users (Public): • Core Functionalities: o Emergency call function with location services for dispatch. o Secure login and registration. o Real-time ambulance/EMS tracking. o First aid instructions. o Medical information storage

Access to on scene of incident care called pre-Hospital in case of A medical emergency. According to WHO, a medical Emergency is An illness, injury, symptom or condition so serious that a reasonable person would seek care right away to avoid severe harm. Standard waiting time between when dispatch receives call and getting to scene is 5 minutes in first Development countries and almost nonexistent in Uganda. According to the Uganda Police Force annual report 2023, the number of reported incidents of road crushed increased by 16% from 20,394 in 2022 to 23, 608 in 2023. 4,179 of these were fatal, 12,487 were serious and 6,942 were minor crashes. With only this being in Kampala, and this was during covid times with limited access, one can only imagine what the need now. The study showed that the majority of government facilities activated the ambulances by a phone call to the driver (68.6%) compared to the non-government facilities which had the ambulances parked at the facility (91.7%).

By leveraging technology and creating a user-centric experience, the Maama Namu app can significantly improve emergency medical services in Uganda, saving lives and fostering a healthier community.

The target market for premium healthcare in Uganda is significant, with a population of approximately 49.25M People, Research conducted by the Uganda Ministry of Health and Ugandan police indicates a rising demand for quality Emegency medical services, particularly among urban populations like Kampala, driven by factors such as increasing urbanization, rising incomes, and changing lifestyle patterns. The Increasing conjestion in Kampala has led to increase incidences of road traffic accidents, The number of casualties from road crashes in 2023 increased by 15% from 2022 to 2023. According to a report by the World Health Organization (WHO) on healthcare infrastructure in Uganda, affluent neighborhoods represent a Total Addressable Market (TAM) of approximately 49.8M however A government report has indicated that the number of Ugandans who are now classified as middle class citizens stands at 15.64 million this the SAM, with The SOM being about 64% based on the same report.

Statistics from the National Emergency Medical Services Strategic Plan 2020/21 – 2024/25 The main pathologies of emergency care based on the burden of emergency medical conditions which in turn determine the national demand for emergency medical services are: • Traumas/surgical emergencies. Uganda is among the top-ranking countries for road traffic injuries in the world. • Road traffic accidents and traumas due to other causes. These accounted for 7% of all causes of mortality. • Obstetric and Neonatal emergencies with an estimated 16 maternal deaths per day and 40,000 still births per year in Uganda. The Maternal Perinatal Death Surveillance Review reports show that the biggest contributing factor to the high facility-based mortality is delays in emergency transportation to health facilities. • Mental health emergencies • Medical/Public health emergencies; Emergency complications from non-communicable diseases such as hypertension, diabetes, cardiac disorders, sickle cell disease.

The study showed that the majority of obstetric emergencies were transferred to KNRH by government-owned compared to the non-government-owned ambulances. Surprisingly, there was no uniform approach or standards followed by both types of ambulances despite recent advancements in the provision of emergency care including the launch of the EMS policy. The study showed that the majority of government facilities activated the ambulances by a phone call to the driver (68.6%) compared to the non-government facilities which had the ambulances parked at the facility (91.7%). The majority of the ambulances recording the shortest transfer times were ambulances from non-government facilities (75%). Most EMS responders think EMS is best overed in hospital but it is most needed at the scene. The majority of the ambulances recording the shortest transfer times were ambulances from non-government facilities (75%).Majority of government ambulances (60.2%) traveled between 7 and 13 km to reach KNRH while the majority of non-government ambulances (45.8%) traveled more than 13 km.the majority of government ambulances (63.3%) did not have medical staff on board compared to 93% of the non-government which had staff on board.The study revealed that 27.9% of government ambulances had nurses as medical escorts compared to non-government ambulances which had 89.4% with a nurse as a medical escort.The Government of Uganda, through the department of EMS, has established regional call centers where a government ambulance can be dispatched to respond to an emergency. However, this study showed that the government facilities preferred to activate the government ambulances by directly driver.
