My solution, a mobile app that serves as a medical diary for patients, is a prime example of how these tools can be utilized to improve healthcare outcomes and enhance patient experiences.

Traditional healthcare systems are plagued with data silos, fragmented data, and a lack of interoperability, which hinder efficient care coordination and accurate diagnosis. As an innovator with a solution to this challenge, I propose the development of a mobile app that serves as a medical diary for patients. When a patient visits a hospital, their medical summary will be generated and transferred to their phone with a passcode for easy access. This will enable doctors in different healthcare facilities to retrieve relevant patient data, leading to better care coordination and timely interventions. Additionally, the app will utilize AI to determine patient prognosis based on their medical history, allowing for more accurate diagnosis and treatment plans. Furthermore, the app will create social groups where patients with similar illnesses can connect, make friends, and engage in regular discussions.

The challenges associated with traditional healthcare systems characterized with data silos struggle, fragmented data and not readily shared between different healthcare providers. This lack of interoperability hinders efficient care coordination, accurate diagnosis, and timely interventions.

The mobile app that I have developed is aimed at serving patients who visit multiple healthcare facilities and often struggle to provide their medical history to new doctors. The targeted beneficiaries of this app are individuals who need a convenient and secure way to access their medical information and share it with healthcare providers.
